This service provides DOT-complaint urine drug screen collection performed in accordance with U.S. Department of Transportation (DOT) regulations.
Collection is conduced following required chain-of-custody procedures for employers, safety-sensitive employees, and regulated testing programs.
This service includes specimen collection and documentation only. Laboratory testing and Medical Review Officer (MRO) services are not included unless arranged separately by employer or testing program.
Appointments require at least 24 hour' notice for cancellation or rescheduling.
Late cancellations or no-shows may be charged the full service fee.
Additional mileage or after-hours fees may apply outside standard service areas.
DOT collections are regulated under federal guidelines. This service includes collection and chain-of custody documentation only.
Laboratory testing, MRO review, and employer reporting are handled by the designated laboratory or program.
No medical advice, diagnosis, or treatment is provided.
Donor must bring valid government-issued photo ID. Employer authorization or DOT testing paperwork is required.
A private restroom must be available at the collection site. Donor must follow DOT shy bladder and observation rules if applicable.
