Mobile Wellness Visit includes an in-home visit with basic health screening services. This service is ideal for routine wellness checks, monitoring, or preventive screening, or clients who need convenient mobile service.
Included Screening Tests:
Fingerstick glucose screening
Urinalysis (dipstick screening)
CLIA-waived point-of-care screening as applicable
Include in this visit:
Mobile service visit
One CLIA-waived screening (choose one): glucose fingerstick OR urinalysis dipstick OR pregnancy test OR rapid flu/COVID/strep (one test)
Proper specimen handling and documentation
This service includes CLIA-waived screening and/or specimen collection only. Test offered may included glucose fingerstick, urinalysis dipstick, pregnancy testing, or other CLIA-waived rapid screenings depending on the service selected.
This service does not include diagnostic testing or medical diagnosis. Results are provided for screening purposes only and may require follow-up with a licensed healthcare provider.
Client must be 18 years or older, or have a parent/guardian present.
This service includes screening and collection only. We do not diagnose, treat, or provide medical advice.
Additional screenings or services may be added for an additional fee.
Service is provided by trained personnel operating under Moorman Diagnostics and Collections LLC protocols.
Appointments must be scheduled in advance and are subject to availability. Clients are responsible for ensuring accurate service location and contact information at the time of booking.
Clients must be present at the scheduled time. Delays or missed appointments may result in cancellation without refund once the visit has started or specimens have been collected.
Rescheduling requests must be made at least 24 hours in advance when possible. Sam-day or after-hours services may be non-refundable.
